How to create a confession page using google docs ?

I wish to create a confession page for my school on fb. I've seen several confession pages but don't know how to create a confession doc. Using google drive ? Need a

MAKING A CONFESSION FORM- Go to https://docs.google.com. Click on "Create" and select "Form" Now choose desired theme and Complete the form. Click on "Add Item" and Select "Paragraph Text" Customize the blank fields and save. Now click on "View Live Form" A new tab containing your form will open up. Copy the URL and shorten it using any Link Shortner like Bit.ly or goo.gl. Add this shortened link to your Facebook page.
